The FAA has produced a useful series of videos entitled the Safer Skies Aviation Training Series. The series originated from a 1998 initiative which identified the most critical and common causes of most aviation accidents. Identified causes included:
- Weather
- Aeronautical decision making
- Loss of control
- Controlled flight into terrain
- Survivability
- Runway incursions
In the first video of the series, weather is discussed and it was mentioned that 27% of accidents are weather related and fully 90% of weather related accidents originate with bad decision making by the pilot. Hence, the nearly hour long video discusses weather planning and in-flight weather decision-making and is well worth watching.
